The PCC is a position funded through the Ministry of Health and Long Term Care. PCCs facilitate palliative care education, consultation and linkages for staff of Long Term Care Homes, Community Nursing and Support Agencies, Hospices, Day Programs, Supportive Housing and other organizations.
The Palliative Care Nurse Consultants serve as a resource to assist staff in the interpretation and application of The Model to Guide Hospice Palliative Care, best practice guidelines and assessment tools. The services provided include telephone and on-site consultation for care providers, problem based education, training and mentorship for front-line staff, and information on current, research based best practice guidelines related to palliative care and linkages to other palliative care resources. PCCs provide consultations to staff. They do not provide direct care.
