In the Seniors Health Research Transfer Network (SHRTN), a Community of Practice (CoP) is a group of people who come together to exchange information and knowledge on a specific topic related to seniors’ health and health care. Please visit our website at http://www.SHRTN.on.ca.
SHRTN CoP members include paid and unpaid caregivers, researchers, policy makers, educators, specialist librarians and others who are interested in the topic.
The End-of-Life Care Community of Practice is one of SHRTN’s 13 Communities of Practice. The purpose of the Communities is to provide palliative care providers with the mechanisms to work in collaboration.
